I recently tried out the feature of adding a hashtag that causes replies to become moderated. I found that when visiting such a pending message on the web, both the "Approve" and "Approve and unmoderate sender" buttons are available, even if the sender is already unmoderated. This gives fellow-moderators the false impression that the reason why the message is pending is because the member is currently moderated.
